What are Blackheads and Whiteheads?

Blackheads and whiteheads are referred to as comedones. On our face there is a lot of fine hair which is called villas hair, there is a gland attach next to this hair and the entire unit is called a pile of the sebaceous unit. Whenever there is a blockage of this hair follicle with either sebum or skin debris or keratin, this leads to the formation of comedones. If there is a thin layer of skin on top of the collection of the skin debris and sebum, then this leads to the formation of whiteheads. And when these comedones are exposed out in the environment, this leads to the oxidisation of keratin which leads to the appearance of blackheads.

 Blackheads and whiteheads by simple definition are black spots or white spots on the face which appear on the face due to the open pores on the skin. The most common areas where they appear are on the sides of the nose, cheeks, foreheads and even on the chin area. Blackheads are open comedones, these are the pores that are not open on the surface of the skin. Whiteheads, on the contrary, are closed comedones they are pores that are opening on the surface of the skin but there is a block on the surface. Hence it appears as a white spot.

These are precursor lesions of acne. You will notice that people who have a lot of acne or have oily and combination skin develop these Blackheads and Whiteheads mostly on the forehead, nose and chin area. Acne lesions are mainly of two types: inflammatory acne and non-inflammatory acne lesions. Inflammatory acne lesions are the ones that appear very red, the papules, nodules, cysts and pustules where you can see these yellow dots. And non-inflammatory acne comprises these blackheads and whiteheads. These whiteheads can lead to the formation of red acne, so need to treat these whiteheads so that do not form these red acne lesions in the future and also and also to reduce the chances of scaring.

Causes of Blackheads and Whiteheads?

Blackheads and Whiteheads are the initial stages of pimples or acne vulgarises as you choose to call them. They usually appear on skin whenever skin is undergoing hormonal changes, that is around the pubertal period. so, it is very common in adolescent children both boys and girls and it can also be seen in adults especially around the menstrual cycle when hormonal changes are going around in the body. They are most commonly seen in people with oily and combination skin and also in people who use a lot of cosmetics. When you apply a thick layer of cosmetics, this will block the pilosebaceous unit and lead to the accumulation of sebum in this area and causing the formation of blackheads and whiteheads or causing exacerbating these lesions. If you apply a lot of hair oil that tends to trickle down on the forehead and sides of the face and leads to the occultation of this opening and it could also lead to the formation of these lesions. it is also seen n people who sweat a lot.

How do we treat Blackheads and Whiteheads?

For the treatment, we need to reduce the accumulation of oil and keratin in the hair follicle opening. You can use the chemical exfoliant, you can use creams and gels that contain salicylic acid. Salicylic acid tends to concentrate in areas where there are oil forming glands. So, it’s very lipophilic. When it concentrates in these areas, it tends to shed the dead skin and lets the oil come out smoothly. This leads to a reduction in the formation of Blackheads and Whiteheads. 

How can you use salicylic acid?

You can use salicylic acid in the form of face wash formulation 2% salicylic acid is good for face wash. Firstly, you should wet your face on damp face apply the face wash, lather it on leave it on for about 30 seconds, try to form a foam on the forehead, nose, chin and also on the cheeks if you have a lot of blackheads and whiteheads in this area. Do not try to rub it, be very gentle with it from foam and wash it off with lukewarm water. This will help in loosening the keratin and reduces the accumulation of sweat and dirt in your open pores. You can wash your face twice if you have oily skin and once a day if you have combination skin and if you have dry to sensitive skin, then you should use this face twice or thrice a week. It may make your skin a bit dry so make sure you apply a layer of moisturiser, you can use a gel-based moisturiser which is non-comedogenic, so it doesn’t block the pores and make your blackheads and whiteheads worst. If you have a lot of blackheads and whiteheads, it is best to visit a dermatologist to tackle this problem because this sometimes leads to inflamed acne and also leave behind scarring.
